(Effective Jan 2015)
This is the estimated costs of the services for the procedure that you are considering: In-Vitro Fertilization – Embryo Transfer (IVF-ET). The fees below are based on completed payments prior to treatment start. As defined by the CDC* and ASRM**, the start of the cycle is when a woman begins taking medications to stimulate egg production or ovarian monitoring with the intent of having embryos transferred. Not all started cycles result in egg production or embryos transferred. Fertility & Genetics reserves the right to discontinue this treatment at any time it deems necessary. Patients are responsible for the total cost of the contract. The total cost may vary based on the specific procedures, tests and other services that may be required for you individually. In addition, any required diagnostic tests and/or services prior to the start of treatment are not included.
